The National Collaborating Centre for Determinants of Health (NCCDH) works to move knowledge into action on the conditions of daily life in which people are born, grow up, live, work, play, learn, and age and the driving forces that shape these conditions – the structural and social determinants of health.
NCCDH supports uptake and integration of relevant knowledge and evidence in public health practice, programs and policy through development of evidence-informed resources, exchanging promising practices with the Canadian public health community and bringing together diverse voices and multisectoral perspectives.

Toward a safe and healthy place to call home
This document summarizes results from a 2023–2024 survey of Ontario public health unit practices to promote healthy housing. Highlights of survey results are presented alongside public health roles and promising practices to address inadequate and unhealthy housing conditions as a determinant of health and key driver of health inequity.

This curated resource list presents 10 actionable health equity frameworks, covering a range of topics where public health action is required (e.g., racism, climate change). The list includes resources on other issues that require framework users must consider such as anti-oppression and critical allyship.
Determining Health: Food systems issue brief
This issue brief describes key concepts needed to understand what food systems are and why they matter for public health in the Canadian context. Drawing on peer-reviewed and grey literature, it further explains how food systems impact health inequities.
